West Feliciana Parish offers our residents a Social Services Department to help you with a wide range of resources. We: • Provide mental health assessments and appropriate discharge planning and education to patients. • Make referrals for needed services such as mental health appointments, home health and personal care attendants, hospice services, and durable medical equipment. • Assist patients with locating valuable community resources such as prescription assistance, Medicaid programs and waivers, Council on Aging services, legal services and more. • Provide crisis intervention and short-term counseling. • Act as an advocate for the patient and a liaison between the patient, their family, and their Medical team to assist and develop a patient’s plan of care. • Oversee patient satisfaction (including surveys) and resolve grievances/complaints within a timely manner. • Provide information and oversee applications for the hospital’s Charity Care program. • Provide education, motivation and behavioral modification to those participating in the WFPH Diabetes Program. • Serve as a resource to the community and provide assistance to those in need.
*West Feliciana Parish Hospital is a Louisiana Medicaid Program Certified Application Center. Our on-site social worker is a trained Medicaid Application Specialist available to assist those in the community wishing to apply for Medicaid coverage.
