West Feliciana Parish Hospital Diabetes Self Management Education Program

Did you know the Centers for Disease Control estimates that more than 20 million Americans have diabetes; however, six million are unaware they have the disease largely because they fail to recognize the symptoms? That's why West Feliciana Parish Hospital has developed the Diabetes Self Management Education Program, which can empower you with the ability to help improve your quality of life. Controlling this disease may seem like a difficult task, but with our help you can learn to make diabetes management a little easier.

How Does the Program Work?

1. Enrollment: To enroll in the program you must be referred by a physician.

2. Assessment: We will make an appointment for your assessment. Our team, including a registered nurse, social worker and dietician will all work closely with you and your referring physician to provide the necessary tools you need to successfully manage your diabetes.


3. Classes: You will attend a variety of classes consisting of two half-day alternating sessions. Classes are held on Thursday's from 8 a.m.-12 p.m.

Diabetes Support Group

If you or someone you know is struggling with diabetes, the West Feliciana Parish Hospital (WFPH) Diabetes Support Group can provide motivation to take those first steps to a healthier life. Diabetes patients need to know they are not alone and there are people who can help them along with their journey. Meetings are held every second Monday of the month from 5:30 p.m. - 6:30 p.m. at West Feliciana Parish Hospital.
